The second step of the survey is to fill out details about your organization. Some of this information has been auto populated from your registration. Review and verify the prepopulated information and provide the additional details requested. If your organization is a subsidiary of or headquartered in a country other than Canada, enter the details for your Canadian operations.

Parent NameTextEnter the name of the parent organization/company. Only required if the organization level selected is not corporate.
Headquarters ProvinceDropdownSelect the province or territory in which the participant organization’s Canadian headquarters is located.
Headquarters CityTextEnter the city in which the participant organization’s Canadian headquarters is located.
Ownership TypeDropdownSelect the statement that best describes the nature of the participating organization’s ownership type from the list provided. See definitions here.
Ownership Type OtherTextIf “Other” is selected for Ownership Type, describe the ownership structure of your organization.
Ticker NameTextIf “Publicly Traded” is selected for Ownership Type, input your ticker symbol.
Fiscal Year StartDate (MM-DD)Enter the participating organization’s fiscal year start date in the format MM-DD (for example, April 1st would be entered as 04-01).
RevenueDropdownSelect the revenue range in which the participating organization’s annual revenue from the last fiscal year falls in Canadian dollars. This field is only required of for-profit organizations.
Assets Under ManagementDropdownSelect the asset value range in which the participating organization’s total assets under management fall from the last fiscal year in Canadian dollars. This field is only required for financial and insurance organizations.
BudgetDropdownSelect the budget range in which the participating organization’s annual budget from the last fiscal year falls in Canadian dollars. This field is only required of not-for-profit organizations.
IndustryDropdownSelect the industry that best aligns with the participating organization from the list provided. If none apply, choose “Other”. See definitions here.
Other IndustryTextIf “Other” is selected for Industry, describe the industry in which the participating organization operates.
Permanent HeadcountNumericEnter the participating organization’s total number of permanent employees at the time of submission. Exclude temporary, contract, inactive, on leave, and student staff.
UnionizedToggle “Yes” or “No”.Click the toggle to move it to the right if the participating organization has unionized employees.
Unionized HeadcountNumericEnter the participating organization’s total number of permanent unionized employees at the time of submission. Exclude temporary, contract, inactive, on leave, and student staff.
Collective Bargaining Agreements CountNumericEnter the total number of collective bargaining agreements in place at the participating organization.
